Nomination Forms
Before making a nomination, it is essential to read our accompanying guidance documents. These contain advice on how to complete each section of the forms.
The proposer (Fellow) must submit all documents by 30 October for a nomination to be considered.
In addition to the nomination forms, nominees also have the option to complete a confidential individual circumstances form, detailing any circumstances that may have affected their career.
There are currently two different sets of nomination forms for:
- Industry, Commerce, the Arts and Professions (ICAP)
- Humanities, Arts & Social Sciences (HASS) and Science, Technology, Engineering, Mathematics and Medicine (STEMM) Fellowship Nominations

Please read the guidance and, if unsure, seek input from the Fellowship Officer to ensure your nomination is proceeding through the correct route. Your nomination will be rejected if it is not submitted to the correct Scrutiny Committee.
